1. What these self-tests are

The questionnaires on this site are recognised screening instruments used in general practice and research. They are offered here for education and self-reflection.

Using this website, taking a test, or buying a report does not create a clinician–patient relationship between you and Insight Inc or any of its clinicians.

2. In an emergency

This website is not monitored, and nobody at Insight Inc is alerted when you complete a test. Do not use this site to seek help in a crisis.

3. Who may use this service

You must be 18 or older to take a test or purchase a report. By using the service you confirm that you are, and that the information you enter is your own and is accurate.

4. The paid report

Taking a test and seeing your score is free. You may optionally buy an interpreted Personal Results Report, delivered as a PDF to the email address you provide.

• Personal Results Report — $14 for a single assessment.
• Full Assessment Bundle — $39.

Prices are in US dollars and include no recurring charge: each purchase is a single, one-time payment. There is no subscription and nothing renews automatically.

Delivery

Reports are generated and emailed automatically, usually within a minute of payment. If your report has not arrived within a few minutes, check your spam folder, then contact support@insightinc.us and we will send it manually.

Refunds

The report is a digital product delivered immediately, so it is generally not refundable once it has been sent. However:

• If your report is never delivered, or is corrupted or unreadable, and we cannot resolve it, you will receive a full refund.
• If you were charged more than once for the same order, the duplicate charge will be refunded in full.

To request a refund, email support@insightinc.us with the email address used at checkout. We aim to respond within two business days.

Payment

Payments are processed by Stripe. We do not receive or store your card details. Your card statement will show the charge from the business name registered with our payment processor.

5. Acceptable use

You agree not to:

• use the site or reports for any unlawful purpose;
• attempt to interfere with, probe, or gain unauthorised access to the site or its ordering system;
• resell, redistribute, or republish a report, or present it as your own work or as a clinical assessment you performed;
• submit answers on behalf of another person without their knowledge and consent.

6. Intellectual property

The interpretation, layout, and written content of the Personal Results Report are the property of Insight Inc. When you buy a report you receive a personal, non-transferable licence to keep and use your own copy. You may of course share your report with your own clinician.

The underlying questionnaires are used as follows:

• The PHQ-9 was developed by Drs. Robert L. Spitzer, Janet B.W. Williams and Kurt Kroenke and colleagues, with an educational grant from Pfizer Inc. No permission is required to reproduce, translate, display or distribute it.
• The ASRS v1.1 Screener was developed by the World Health Organization in conjunction with the Workgroup on Adult ADHD and is made available for screening use.

7. Availability

We aim to keep the site available but we do not guarantee uninterrupted service. We may change, suspend, or withdraw any test, report, or price at any time. A change in price does not affect an order already paid for.

8. Limitation of liability

The self-tests and reports are provided on an “as is” basis, for informational and educational purposes only, and without warranty of any kind. To the fullest extent permitted by law, Insight Inc is not liable for any decision taken, or not taken, on the basis of a score or report from this site, nor for any indirect or consequential loss. Nothing in these terms limits liability that cannot lawfully be limited. Where liability cannot be excluded, it is limited to the amount you paid for the report in question.

Nothing on this website is a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified health provider with any questions you have about a medical or mental-health condition.

9. Privacy

Our handling of your information is described in our Privacy Policy, which forms part of these terms.

10. Changes to these terms

We may update these terms from time to time. The version published on this page at the moment you place an order is the version that applies to that order.

11. Governing law

These terms are governed by the laws of the State of New York, without regard to its conflict of law rules. Any dispute arising from them will be brought in the courts of that state.
